Yes it harms the baby. Narcotics (codeine) that is in Tylenol #3 can cause fetal nervous system depression, drug withdrawal, reduced birth weight, and using them during pregnancy is not recommended. Tylenol (plain) is OK to use during pregnancy if you use less than 4000 mg daily.

It depends on the age,weight and dosage. Other than that you should use regular tylenol for the baby.

Any drugs doing pregnancy can harm your unborn. If you are living with pain that reqires you to take such a strong drug as codeine, I really suggest to you to talk to a herbalist about homeopathic remedies. Even meditation is a better solution. With so many variables of children's immune system, you really can't take chances.
