So you are going to refuse a medically-necessary medical procedure in an attempt to get a medical discharge.

What you are going to do is wind up with a permanent injury to your arm and a 'non-service-connected' rating from the VA.

After your surgery your doctor will give you a profile. This will say what you can and cannot do. Give a copy of the profile to your 1SG and talk to him about it. This way, if somebody tries to make you violate your profile you can go to top and get him to straighten things out.

I could be wrong, but I don't think one's alllowed to refuse. Army docs being officers; their decisions are seen as orders. I could be wrong, so keep lookin into it. A medical discharge will make it a LOT easier to get post service medical from the V.A. Even with valid L.O.D. paperwork, stickin it out for an honorable will make things more tedious. Getting back in can be tougher though, with a medical.
Can say that from xperience. Even if every screw and pin in my legs were labelled, U.S.GVMNT I'd have the extra layer of beaurocrocy in the V.A. to wade through.
