Yes, you should take off your cap before you sit down to eat, and perhaps go wash your hands and comb your hair as well. A true gentleman never wears a hat indoors.

depends on if you'd feel uncomfortable. If its a fancy restaurant than you should've left it at home in the first place, if its a cafe or burger king, than you shouldn't be too worried, but normally a gentleman is required to take his hat off no matter what the building or place. so it'd be better just to take it off. all depends on comfort otherwise.

Hats, as well as any other headgear, save the orthodontic type ;) should not be worn at the table. Doesn't matter if it's nice or not, it's just considered rude to eat with a hat on.

A ball cap should be removed at every dinner table, not just at a restaurant. (Proper etiquette)

It depends on the restaurant. If its fast food, it doesn't really matter. Anywhere else, regardless of what kind of hat you are wearing, it is polite to remove it.

Men should remove their hats while eating, In Christian churches, funerals, taverns and hospitals unless they are Military police on guard duty. Women should have hats on in these places.

I think it depends on the restaurant. If it is a top notch place that is known for elegance, then yes.
