If you are in the US, report it to the police, then to the US State Department, and/or go to the Post Office where you can make an application and ask them what to do. Yes, you'll have to pay the fee again.
If you are not in the US, then report to the nearest US Embassy or Consulate. Unfortuantely, it's going to be extremely difficult to get another passport if you are overseas, unless you have your original copy of your birth certificate with you, which is unlikely. I don't know what they do in that case.

You should contact also the passport office where is was issued as passports are coded so that code can be tagged with stolen and it would mean that it is invalid
Yes they will charge you around 40 quid!

If you are out of the country, go to an american embassy immediatly. Yes, you will most likely have to pay to have it replaced. Report the loss to the credit unions and to the FTC.
