you can contact the RIBA (royal institute of british architects).
They have a very good advisory service and can put you in contact with architects that do this type of work regularly.
You can also go drirectly to a number of builders. Inivite them to look round the house and tell them what you're after. They can then all go away and prepare some basic costs / plans etc. for you - you can then decide which to go for.
Go for reputable builders and get a contract signed with them - something like the JCT (joint contracts tribunal) 2005 form - either small works or intermediate works contracts will be suitable.
I would always go with an architect where possible - particularly for such extensive work - they will know all the building regualtions, fire regulations etc.

OK your first step to which i would advise would be call the consumers they have lists on who is recommend for value and quality of work. Also you will have to apply to the council office on any repairs that are structure and extensions i believe, so just in case give them a call. I know it sounds like alot of work but it two calls and one headache gone. You can call directory enquires for numbers of domestic architects and then phone around getting quotes. Once you have your design and are ready with the build get a solicitor to draw up a contract with the builders, this means that if they go seriously over a time they gave or over the budget you will be cover, many builders go over budget by five ten grand, but just in case they try to get more the contract will put them straight and you wont have to pay for work they didn't do unless you change the lay out plans. When it is done your be happy with it and your soon forget the hassle and the dusk and enjoy your new look. Hope this helps.

Go ahead and have some builders take a look at your project. Most often, a reputable builder will have architects they work with frequently on the type of work they specialize in. So, if you find a builder who will do this type of remodeling, he'll know which architects to put you in touch with. Additionally, your builder will know exactly what local protocols and ordinances to follow. He'll be able to tell you if you project is within local codes, and give you lots of advice.

Have a look around your area to see if anyone is haveing any building work done. This way you get to see the quality of work, and any builder worth his salt will gladly take time out to discuss the work you are talking about. If you live in the London/Essex area I know of a very good builder that did all my work.

get some home design software
decide exactly what you want to do and don't change your mind
try to find someone to be a Project Manager
alternatively - take it slowly, do one room at a time and manage the tradesmen yourself
(are you SURE you want en-suite? I find the sound of my beloved on the toilet distinctly unappealing!)

You need to get planning permission. So like the council if you live in the UK, but in America don't have any idea.
