Edited to add: I gave my little sister a digital scrapbook. I took a bunch of pictures of us together and made scrapbook pages, added some music and made her a DVD for her.

If she has kids offer to take them for the weekend. If she works offer to do her housework for a week. Everyone can always use a break from routine sometimes.
