Go to the health department in the city you were born and they can issue you another birth certificate. You have to pay a small fee for it.

If you are in New York you can go New York State Department of
Health, 800 North Pea rt Street, Men ands NY 12202
you will need Your Drivers License or Passport You can also use any other Government photo issued ID the Fee is $30 , good luck

You should be able to go down to your county clerk's office and get a copy of the birth certificate. I don't know what it would be called in New York, but where I live, we call it the county clerk's office. I found this link that might be helpful to you.

Do you have any document that proves you are here and alive :)...like your social security card?
Recommend you go to the website for NY State (Birth Certificates) and see requirements.
http://www.health.state.ny.us/vital_records/
Once you get all the info together, request at least 3 copies of the document. Keep one for yourself (in a safety deposit box, if avail. Give one to your parents to hold or someone you trust)

contact the county seat the you were born in and they should have a copy. For a fee they will send you a copy of it. If that does not work contact the hospital you were born at.

the county courthouse where tou were born go to liscence dept (marrage ect) here in Washington state they charge $17 for a certified copy good luck

Go to the county where you were born and find the county medical center and just give them an ID and fill out the aplication and wala!! the will print you one out
