you may be referring to a Financial Statement which is something that both parties have to complete when involved in a family matter asking for (or contesting) child support and/or spousal support.

both the payor and the recipient must swear a financial statement as to their income, expenses and monthly surplus/deficit, in an effort to determine how much support is payable.

I don't know which country you belong,but by and large, there is no financial affidavit.However, generally speaking affidavit is a self proclaimed legal document on any issue. Suppose you have lost your birth certificate, you can declare on a stamp paper the age and take responsibility for its bonafide nature and that document is called an affidavit...It can be on any issue..
