the "hand of God" has blessed Priests, so it's kind of a metaphoric symbol.
Its not any more unsanitary than kissing someone that you know's hand.

And how do you know where your boy friends/girl friends lips and tongue have been when you kiss them.......
When we do this "symbol" kissing of the back of the hand, we don't slurp our lips and saliva all over the place.
