Uh YES in a big way.
#1. She could end up pregnant and a baby born of a brother and sister can have a lot more health problems or end up deformed.
#2. Girls put a lot of emotional energy into sex and this could really, really hurt her.
#3. It could be very weird for him when he is dating other girls to have them near his sister. Heck, this discomfort may last the rest of his life!
You need to tell their parents if he hasn't already. I know ratting out a friend is hard, but when a friend is doing very dangerous things, a true friend is going to do what they need to, to make them safe again.
Tell the parents. TODAY!

The records show that out of 75% of all incest cases in the united states that approximately 3% are reported. Guess it depends on what side of the percentage you are on.
