There are hardly enough fellowship positions for Canadians let alone internationals. The whole process of becoming a physician is a pain in the butt in Canada because it is crazy competitive for only a very limited number of spots. This extends all the way down to med school acceptance.

Just to get into ANY med school your looking at a 3.85 GPA and at least 30Q on the MCAT.

Things however are changing slowly, I assume that in the future more universities will be welcoming internationals as more physicians retire in the near future. This includes fellowship positions and general acceptance into Canadian med schools.
