I assume by "empty injection" you are talking about injecting a syringe full of air into a system. This is called an Air embolism, otherwise known as getting gas bubbles trapped in your bloodstream. A gas bubble does not always necessarily cycle through your bloodsteam, but may lodge itself in any blood vessel.
The pain can be from as little as a skin rash or an irritation, to a complete acute heart attack if it goes into a coronary artery.
So no, getting a gas in your bloodstream will not necessarily cause death, but no good can possibly come from this.
The only thing that can come from the situation you've asked is nothing but harm.

Injecting air into the bloodstream causes an air embolism and will cause extreme damage if you are LUCKY. It often causes death. Note well, the cause of death is detectable and it will be traced back to the needle mark. It by no means close to the perfect crime (murder). (it's been tried).
Forget it. In some states mere possession of a syringe is a criminal offence if the possessor has no legitimate use for it. (and a prescription, e.g. for diabetic insulin injection).

It can cause death. That's why they make IVs drip, to ensure that no air gets into the blood stream.
Other substances, I would imagine, wouldn't exactly helping or healthy

If by this you taking a needle and pushing air into their bloodstream then DO NOT DO THIS! This is very dangerous and could lead to killing that individual.
