Everytime me and my girlfriend put the condom on, air bubbles appear inside the condom. We are following the instructions completely. This is our first time, so we want to make sure that it is ok or not.

Put a couple of drops of KY lubricant in the tip before you put it on and rub that part around the head a bit. It will probably eliminate the air bubbles. It will also make things feel better because you will git a little bit of movement sensation around the tip.

Air bubbles are fine, holes are bad, and the condom should fit snugly. The end SHOULD have extra space, not air, so that the sperm has someplace to go.
