How do you know humans have been smoking since the stone age? Evolution is a funny thing -- we aren't able to predict the route that mutations will take (and evolution is based on mutation) so we just have to deal with what comes along. We haven't been able to evolve to avoid ANY illnesses, or plagues. The only reason we are able to survive today is because for some of them, we have been able to find antidotes, preventatives (immunization) or cures. But that has nothing to do with evolution.

We haven't evolved to be able to fall off a cliff and not break a leg, either. Some things evolve, some don't. Any time you put heated garbage in your lungs you are going to suffer.
