p.s. i have experienced all of the above and finally came to the REALIZATION that alcohol had alot of do with my PROBLEMS.

1. Every time I drink
2. Every time I drink - but I've learned what liquors cause more !
3. Too sick to show up for work - or left early because of throwing up !
4. What am I doing right now ?
5. I'm happy about this one ... I don't have any DUI's and don't plan on getting any in the future !
6. Sent myself to the hospital twice when I lived in Germany - drank too much !
7. I'm pretty close to my family so this one doesn't count.
8. I'm a happy person so even drunk I'm crazy and happy !

And I know I don't have a problem ... I've learned from the above and know how to handle my drinking.

Not number 3 and I don't know what number 5 is but I came to terms with my alcohol problem last year and it was the best thing that could have happened to me, I had a massive fight in public with my b/f, both of us were drinking and I woke up the next morning so depressed I wanted to die. Right then and there I knew it was time to give up. I did not drink every day or binge drink but I still feel that I cannot drink safely so now I don't drink.

Change ones look - No 5.Appear in a commercial (excludes promoting upcoming CD release) - No, unless said commercial is just ridiculous 6.Having a song appear in a commercial - No 7.Having a song appear in a video game - No 8.Having a video game based on the band - Yes 9.Doing a CD of Cover Songs - No (but a little disappointed) 10.Only allowing a specific retailer to sell your upcoming release - No if first offense 11.Appearing as a guest musician on someone elses release that performs in a completely different genre (If, for example a metal singer or guitarist appeared on a i/e Rap/ Country/Pop album) - See #9 12.Appears as a special guest on a tv show (i/e sitcom, drama etc)(excludes talk shows) - Depends on the show 13. Congrads on the realization! Some people never come to that and some it takes many, many years and alot of problems from alcohol. Glad that you have seen that it can cause problems and lead to many other problems. Good luck to you!

I've never experienced any of those...but have witnessed it too many times with friends/coworkers/family.

Way to go on recognizing thie problem...that's the first step!
