The exam gives a standard, easily checked way for the instructor to evaluate the information gained (and spit back out) for all participants, supposedly without prejudice. It sounds as if you have test-taking anxiety, which is not uncommon, but is self-treatable. Realize that anxiety over being evaluated doesn't magically end with graduation, as we are evaluated using such tools across all life realms, throughout our adult lives. On the upside, realizing the issue lets you begin to deal with it and overcome it's debilitating effect. Perhaps if you envision the VERY WORST that could happen (failing the test/course), and make peace in your mind that you may have to take it over, it can help you to relax enough to let you learning successes shine through on the exam. Good luck, and try to remember it is just one test, NOT the only predictor of your future happiness.
