You get a form to fill in and it will be sent back to a local address and about 8 weeks later you get a certificate hopefully says lots of 'none recorded' in the boxes for the different crimes committed! My employer paid for mine but think it varies for the type of job. (ie full employment cost more than voluntary work)

You need to contact the Criminal Records Bureau and apply for it. There is quite a long form to fill in and there is a charge (around £35 if think). But please be aware that it does take quite a while to come through especially in the summer months as Newly Qualified Teachers and Teachers starting at a new school or just starting the new school year will all be applyig for them. It is the busiest time of year for the CRB. There are two tyypes of disclosure you can get, standard and enhanced. The enhanced one costs more and is generally the only one accepted by schools, colleges and any other careers where you will have regular with children and also by any legal profession, Police, Probation, Courts etc.

Not sure what you are looking for, but as for Criminal Backgrounds Report you would only find information for the area you reside in from your local Police. If you need info from another area (ie city or state) you would have to go to the Department. As for National Crime Information, you will not be given this as a general rule, only law enforcement and only when you are involved in the crime. There is usually a cost for local records checks.
