As a former US Army officer and having recently returned from working with the troops in Iraq for 18 months I can assure you that every single commissioned officer (and NCO & enlisted member) wears a set of Dog-Tags.
For each service they are a little different regarding the information stamped on them.

US ARMY tags contain - example
Wilson (surname)
Richard A (first name, middle initial)
222-444-5678 (social security number)
A POS (blood type)
LDS/Mormon (religious preference)

US NAVY tags contain - example
Benson
Richard J
23 NOV 1950 (date of birth)
Commander (rank)
18 May 1992 (tetanus shot date)
AB - (blood type)
Catholic (religion)

US Air Force tags-contains at least, perhaps more
Campbell
David F
234-456-1234 (SSN)
O (blood type)
Lutheran (religion)

US Marine Corp tags contain - example
Temple
Steven S
557-999-0770 B + (SSN) (blood type)
USMC M (service) (Gas Mask size)
None (religious preference)

All military members wear a set of 2 tags. In the event of death while on duty (killed in action, etc), one tag stays with the soldier, sailor, marine or airman as identification. The 2nd tag is normally secured by the officer or NCO in charge or on the scene for various administative reasons. To my knowledge the only service members who do not wear their dog-tags are the special operations people (SF, SEALs, Delta, Rangers) who are out on classified operations.

I hope this information is sufficient to answer your question.

Yes they do. Especialy in a war zone. It is there to identify them in case they are killed or injured in a battle. It has the name, rank, serial number and on some what kind of medicined they are allergic to. Dog tags have been around since like WWI

We wore 3 dog tags; 2 around the neck and 1 in our boot laces. That way, if you got vaporized by a land mine, the one thing that was usually found intact were your boots (it's true). One boot + a zillion body parts = positive identification + a letter home to the folks (ttttttttttttthats all, folks!!!)

All personnel who deploy are required to have dog tags. The dog tags have the following information
1) Name
2) Branch of service
3) service number
4) blood type
5) and religious preference of wearer (if any).

Correction: Civilian contractors are not required to have or to wear dog tags. I was never issued them when I was a civilian contractor in Iraq, nor was anyone I knew. We had all the ID we needed without them.

Name, SSN, Blood Type Religious preference (though I have seen some soldiers specifically request not to have this)

There's no rank because you'd have to keep getting re-issued everytime you got promoted.

The tag is to ID you so it has your name and your SSN is what they use for ID now.

Blood Type is if your injured and need blood transfusion.

Religious Preference is so they know if you need last rights or any special preparation for burial.
