what do i put on the line with the M?
do i put a number on the line before accepts?

this is an odd choice. Check how they wrote your name(s) on the envelope. The M___ should be completed as "Mr.", "Ms.", etc., then your name(s). The second line is for a check mark and not a number (You should not assume that you can bring guests unless the envelope was addressed to you "and Guest")

You fill out your names

Mr. and Mrs John Smith

and put an X next to accepts with pleasure or next to regrets depending on whether you're attending or not.

You don't put the number of people attending you put all the names on the line, keeping in mind that the only people invited to this wedding are those whose names are listed on the inside envelope.

The M is for Mr, Miss, Ms. or Mrs. For example, if you are a couple, you would write Mr and Mrs John Smith on that line and then x the second line "accepts with pleasure."

You fill in your name (Ms Jean Smith or Mr. John Jones).

If the invitation was addressed to you and a guest, or specifically mentions two people, you can put both the names.
(Ms Jean Smith and Mr John Jones or Mr and Mrs Smith)

You DO NOT add a guest for yourself (or your children). The invitation will indicate whether you are being invited alone or if you may bring a date.

If you are married it would be Mr & Mrs Jones, if you are going with a date and you were invited it would be Ms Jones and Mr. Smith. Single by your self, Ms. Jones. The other line is for how many are going. Have fun at the wedding!
