you just know. usually there's an akward silence if its a special moment or something. if that doesn't happen, then you'll just have to guess it yourself.

Depends how fancy the kiss is going to be. If it's just going to be a (more or less) chaste peck on the lips, then you may not get any warning at all. I've had that happen a couple of times when I didn't really want that - Polish guys don't know that Americans consider even that to be non-platonic.
If he is fixing for something more elaborate, it depends on the guy. Some guys are slobs and won't give you any advance warning of that either. Slobber, slobber. Yuk. If he's more considerate, he'll probably move his face towards yours, stop just short, then look questioningly into your eyes.
And you will no doubt reply. And I hope you will ask yourself what kind of relationship you want with the guy before you're with him. If you don't want a permanent relationship, better to not lead him on, and just keep it platonic. Or if you're not sure, don't know him well enough yet, then you say, "Hey, why don't we save something for later, all right?"
On the other hand, if you do want a permanent relationship and that's what it means to you that you're willing to kiss him, better to discuss that and get it out in the open before kissing him... Wouldn't be too fun to kiss him with those intentions, then find out his intentions were light years apart from yours, right?

this is the most perfect one---he leans in closer to you. u'r both standing. he looks into u'r eyes, then down to u'r lips, then back up and leans in. u start to lean in too. then before u know it, u'r both kissing with his arms around u and u'r hand on his cheek.
