if she is not from the US then she will need her passport. you should also check out the Canadian immigration online to see if she needs a visa.
if you do not have a US passport then you need to do the same.
also check both of your US visas to be sure that you have multiple entry visas for the US or you won't get back!

definitely a passport, summer clothes b/c right now in Ontario it is around 30F, depends where your country is and if she speaks English, but maybe a translator?
