OK, well first do you really want him back? Are you just remembering all the good times and not the bad? Sometimes we do that when we remember back on certain things in our life.
Now, if the answer is yes, that you really want him back, be honest with him. I would call him, keep in contact and drop hints and see how he reacts.

Well i think flirting with him and going out and having fun in front of him will work. Well it did for me!
I used to show up at the same bar as him with my friends and flirt with him. I would dance with other guys and he would stare at me and i would lick my lips and totally come on to him. At the end of the night he would wait around to see if i would go home alone. Then one night i got real drunk and he told my friends that he would take me home. And that was it!
It worked for me i got him back.
Good luck, it's hard but if you really want it work it!

if he cheated then dont, cheaters are always cheaters. but if you really want to get back with him, be his friend and make him like you again like the first time the two of you met.
