About the only time I can think of when you'd leave the "the" out is when you use the words as an adjective:

The " United States," or The " U.S." by the way theres more information that goes along with answer one, The " United States," if somes should happen to ask the united states of What? Then tell them America.

You say 'The United States of America' because America is not the only country to have united states. USSR, the United States of Soviet Russia, is one example. So you have to be specific.

Its the "United States of America". Lots of other countries have used "United States of XXXX" as their name - e.g. United States of Belgium (1790), United States of Colombia (1863-1886), United States of Central America (19th century), United States of Venezuela (1864 -1953), United States of Indonesia (1949 - 1950).

P.S. America is the landmass including Canada and Mexico and C & S America.

It depends on the sentence. You would say, "I am from The United States" I myself would add, of America! I think most often
you would say "The United States". God Bless America!
