Not through the whole thing...just the beginning and then we get used to it...and it becomes enjoyable....well for me at least.....but some girls say that it hurts them the whole time. It really just depends on the girl i think.

Not always. It depends on the girl, whether or not her hymen is intact, and on how gentle the guy is. My first time wasn't painful at all.
