If you'll report it in writing you'd say: Je voudrais signaler que mon telephone ne fonctionne pas.

If you're reporting verbally, this would do: Mon appareil est en panne.

I agree with version of 'jennferbu19'. The word "que" is very needed here. And the end of the sentence did not look like a phone had accident (en panne), though probably it may be said this way too.
