I don't think there is any harm in having such fantasies as long as it remains a fantasy... but during a married relationship and that too about her twin sister... very risky grounds ... based on personal experiences I can say whats more important is that you communicate with her on this topic ...
tell her about your sexual fantasies when you guys are hot n horny ... prior to that make sure the setting is right, she is slightly tipsy (so she can be uninhibited and responsive), both of you are comfortable and not in any rush ... when discussing such matters don't tread on dangerous grounds immediately ... go gradually, checking her thoughts out on the way and listening to her ( most important) ... you will get all your clues to how interested she is during this conversation and you will also get to know what turns her on most ....
Now, once you have that basic idea, steer the direction of your conversation to helping her fulfill her deepest, most obsessive, recurring fantasy of all time... tell her its okay to have such thoughts .. and that you are glad you talked ... from there onwards its up to you ...you can tell her about your favorite fantasy involving her and another woman, and offer to help her live out her fantasies ( which is only fair but beware, be acceptable and be chill) .. your fantasy is about to come true and so is your wife's, you should be ok with that, right? !!!
