Yes it is proper to sign with the initial then the middle name. My father is known by his middle name and signs his name with his first initial then his middle name. I also have a doctor friend who signs his name the same way.

I do. But my mom chooses to use her middle name first and her first initial second... I find it's rude for someone to say,,, but what's your real name? My middle name is as real as my first. It is the one I go by, and I don't know why people have to get all bent out of shape about it. I was giving birth, and my danged doctor called me by my first name, even tho she had called me by my middle name the whole 9 months i went to see her... it sux, but I guess we just have to deal with it, however we choose.

If it's a legal document, then yes, you should use your first initial, middle, last name. If it's regular correspondence, then it's perfectly acceptable to use your middle, last name only.
