During any kind of sports or recreational match, it is against the rules to hit or push below the belt. For example in boxing, if you punch somone below the belt.
From that, the term "Below the belt" means anything thats unfair or is a "low blow"

I think it comes from boxing as they are barred from hitting or punching below the belt line or waist and it is used in everyday language to mean it's just a cheap shot at somebody,especially when calling someone.

it's a boxing term. you can't hit the man below the belt line...for obvious reasons. They call it a "low blow" , so that term probably can be used anywhere to say that was a cheap shot.
