"fan" means girlfriend or boyfriend, but does this mean someone you have sex with only, (as in a lover or spouse) or can it also mean just a friend of the opposite sex? and is this a polite or correct word to use for a spouse?

Depends on which tone you use & which vowel sound you're referring to with the letter "a".

"Fan" means a boyfriend or girlfriend--as in a romantic relationship one.

You would not call someone your "fan" if this person is a boy friend or a girl friend. (A "girlfriend" in an English sense could mean a very close GF (to a girl)—it does not means she is your 'fan'.

Think of it as calling someone your "Boo" (Beau)—like, he's my Boo.
