confine the kitten in a small room with the litter box to reinfoce the habit.. this may take a week or more depending on the age of the kitten - kittens shouldnt have the full run of the house untill they are 12 weeks of age, before that they need to be confined where the litter box is for most of the day, and for sure at night.
a general rule is ONE LITTER BOX PER CAT, and ideally 1 extra box.
It could be the other kittens have bullied her NOT to use the box.. or the location could be bad.. if its nex to the furnace or laundry machine the cycle change could have frightened her...
she is smallest - so if from the same litter she might be a runt with something wrong? has the vet checked the kittens?? I would hope that if they are pets for kids they were dewormed and vaccinated before you got them and vet checked then too.. you would hate to get kittens and have them all get sick and die... so I would assume the vet checked her too to rule out any big problems
Occassionally kittens do have real problems and sadly euthanasia is the only option.. your son can adopt another kitten (if these ones were from pet store I strongly suggest not getting the replacement from the pet store.. because pet store kittens often have behavioral problems...) also kittens from farms often have litter box problems so preferalby not a farm kitten either.. best place is always the SPCA.. and if you got her from the SPCA they would probably have given you some sort of health guarantee...
good luck... (kids learn about human death and how to deal with it, often by experiencing pet death)
age of kitten?? you didnt say if they are over 6 months it could be territorial marking, make sure the females are spayed at 6 months and males are neutered at 8 - 10 months
*** ADDITIONAL INFO ***
"sticking their nose in it" does not work and may even confuse the kitten worse.. so dont do that.. best to retrain and have proper amount of litter boxes

put her in a crate for animals and put a litter box in there and she will eventually use it once she figures out that the litter box is the only thing in there.
I had the same problem, that is what I did and my kitten learned how to use it in about 1 week,I was pretty impressed,so I told my friend about and she did it to,and it worked on her kitten to!
And how ever you tought the other 2 how to use the litter box, teach that one the same way, unless the other 2 kittens just learned on there own.
and if that don't work try doing this, everytime the kitten does use the litter box, (if it will) give it a treat after it does, and it will think,"Hey,I guess now everytime I use this thingy ma jingy I get a yummy yummy treat!"
and if these don't help you out very much try this www.kittentips.com that might help out,and if it doesnt go to www.google.com and type in " How do I train a kitten to use the litter box?" and it will probably give you some good answers
Good l_uck on training your kitten, and if you still have trouble, ask a vet,or a person who knows very much about animals, who can tell you what to do and what not to do.
Oh,and another thing, when your kitten uses the bathroom in the floor or furniture, don't scream at her it will just scare her and then she will just be wondering what happened, and don't spank her on the bottom she will just get mean, and remember that and will still use the bathroom on the floor / furniture.
Hope I have been alot of help to you,and your kitten,Good l_uck again!!!

You can buy a spray at pet stores for this problem. Also as another poster said you might want to get a liter box for each one. A lot of times Cats and Kittens do not like to share because of territory issues. Keep them in different places and scooped out so that smell is not an issue. That should help. IF not you might try to isolate the kitten in a bathroom for a couple of days with the litterbox. I had to do this with my first kitten and after a few days she got the jist of it and was not a problem after that. I recommend using the first two before the latter.
Also some cats are very private so if you do not already have one get a covered litterbox with a door. My older cat will not use the litterbox if someone is in the room (since the door accidently got thrown out). She will actually get out of the box until whoever is in the room leaves. I have actually watched her do this.

There is training spray you can buy at petsmart. It works well for keeping animals off areas you don't want them to go on. I used it to train my kittens. When you see her going say no and put her in the litter box. It takes time and patience, but don't give up.

Put a litter box in the place where she goes. Also, whenever you catch her going, put her in the litter box. You have to be patient, though. Luckily kittens learn much faster than human babies.

you need to scoop up the poo and put it in the litter-box. next step is to put the kitten in the litter-box and use its paws and scoop litter on top of the poo. then u need to find a spray than keeps the kitten away from the furniture.

when having multiple cats it a good idea to have more than one litter box.
The cat may not like the smell of the other two cats.

sometimes its hard to break a pet of old habits, there is this stuff though that you can buy at the pet store that keeps animals from getting on furniture, its a spray u can try to spray the area and keep them away from it also try to put her in the box every hr til she learns to use it, or when u think to put her in itbut taking her back to it will eventually teach her to go potty there

Stick her nose in it and take her to the litter box.
This works although it may seem cruel at first.
