I am leaving the country with my kids with permission from my husband, and I need a disposition for immigration to say he has given permission,
I tried the INS website, no avail,
thanks and any help with this would be great

You can use this form....you have to fill it out, but you also have to pay for the finished product and have it signed and notarized by your husband ....I had one that was free but I cant seem to find the link right now....hope this is what you're looking for

The safest way to precede is to get a court date for a hearing in family court in the county you reside. Then, with your husband present, let the plan be made legal so that it will be legally recorded.
It is dangerous to take your children away from their dad without expressed legal consent and judicial authority. You do not want to be held for questioning at a border !

Make sure he is not B.S. you Call INS see if true get form number. Who told you that
Correction maybe because you don't have your permanent green card this might be the form took awhile to find. Double check with INS Note fee $170 form I-131

I dont think that it has to be a special form, it may just be a notorized written statement from him giving you permission.

Your trying to leave the USA? Don't you mean that you need a passport? You don't need permission.
