An Apicoectomy, or Root-End Resection, is the removal of the root tip and the surrounding infected tissue of an abscessed tooth. This procedure may be necessary when inflammation and infection persists in the area around the root tip after root canal therapy or root canal retreatment.

Apicoectomy is an in office procedure performed by an Oral Surgeon or Endodontic specialist. The area is numbed with a local anesthetic and you may have IV sedation to relax you. Pain medication will be prescribed to keep you from experiencing any discomfort. The area may be sore for three to four days. The gum is reflected (lifted) from an area adjacent to the root area of the tooth. Access to the root tip is then made by removing the thin bone covering the end of the root. A small part of the the root tip is then removed and the remaining root is checked to make sure it is sealed. Apical or Retrofilling (reverse filling) will be required to seal all canals that are found that are not properly sealed . Without proper sealing of all the canals present in a tooth the procedure will not be successful.
The area around the root tip is then cleaned and curetted. The gum is placed back into position and sutured (stitched). After healing all infection at the tip of the root should disappear and bone should heal to fill in the area around the root tip.

It is a procedure where the tip of the root is cut off and then sealed with a root canal sealer and filling, it is like a reverse root canal from the tip of the root down vs. from the top of the tooth down usually done when a root canal fails and the tooth re-abscesses. They do cut into the bone and I would suspect you would be sore for a few days, but things in the mouth heal quickly. Whether they put you to sleep or not would depend on the Endodontist you go to, they do numb you up for the procedure that is for sure. An Endodontist does have to do the procedure, it is one of there specialties.

There will more than likely be some discomfort following the procedure which your dentist should give you a pain medication for. Make sure to take it as prescribed. Also, you can expect some swelling to occur... for this use an ice pack for 20 minutes on and 20 minutes off.

Do they cut into the bone? how long does it hurt for? i have an appointment to c an endodontist and if thats what i need, can an oral surgeon do it?

Yes, I had that. It is local anesthesia, it drills a little through bone. I didn't have any pain but it takes a while.
