Of course it depends on what you call healthy and I made the assumption that I had salt, pepper, and mayo already. I got all these prices where I usually shop (Safeway) using their online catalog at safeway.com. Your question doesn't say how many meals this would cover or for how many people. (I went over by 2 cents)
Groceries with $20
I’d buy:
3.25 lb whole chicken 2.57
5 lb bag of white rice 3.49
1 lb broccoli crowns 1.79
4 Roma tomatoes 1.20
Bunch green onion .59
Pound of whole mushrooms .70
Large Lemon .69
1 lb butter, unsalted 2.99
1 lb cottage cheese 1.50
Loaf of multi grain bread 1.50
Mango 1.00
Plain yogurt 2.00
Roast the chicken with the onion, lemon, butter and mushrooms
Steam the broccoli
Cook rice as directed (steamer or stove top) and serve with a bit of butter
Dice the mango and serve with plain yogurt
Use the chicken thighs, legs, wings and back for this meal
Save the breasts to make sandwich the next day
Cottage cheese and tomato is side dish for sandwich or as a snack

I would buy:
bread - some stores have "day old" bread for a cheaper price.
canned baked beans which come on sale quite often
hamburger
macaroni
canned tomatoes - larger cans work out to be the cheapest
cheddar cheese
You can make a variety of dishes with these ingredients

bread, hamburger- family pack you can divide into 5 or 6 1lb servings, veggies in can, garlic toast. have had to do it before.. i have a recipe for hamburger that will get you several meals if you want to cook all of the hamburger then freeze it into servings or divide then freeze. let me know.. samiam36@mchsi.com

tomato's, garlic, onion, cilantro and some ground beef and a chicken. I could cook almost anything with these ingredients; keeping in mind I have spices at home.
