could be several things best to ask the dentist!

but could be reactions like "keratosis" which can appear eg on areas he has no teeth and is using his gums for chewing, "smokers keratosis" which will dissapear after he stops smoking, or a reaction called Lichenplanus which again should be kept under review.

Anything that looks suspicious will be checked out by oral/medical specialists and if was concerning dentist he would have him refered for a biopsy. they know what they are looking for and will advise him if its serious

(dont mean to firghten you, basically just saying if it was bad he would have been fully advised about this at the time!)

You sure it's on his gums? if it's on the gumline then ... HE'S NOT BRUSHING HIS TEETH!

This causes:
-Plaque Build up, which causes,
-Permanet Stains, NOTE HOW I SAID PERMANET (even if he whitens his teeth the stains will only get brighter and stand out more), and if poor hygiene continues,
-Tartar

But if it is on the gums then I can't help u there.

Could be a fungal (yeast) infection of the mouth. Has he been on any antibiotics lately?

it means the bloods not getting to these bits for some reason and he need to see a doctor.

or he just ate some bread and he need to brush his teeth.

sounds like mouth ulcers, ask your pharmacist about it (if this is the reason you can get creme in the chemist)
