Hi Larry, you're probably talking about an IFR clearance (which I'm not rated in) but my instructor taught me something for ATIS - IWAR - Information, Winds, Altimeter, Runway in Use. Kingschools communications video has a segment showing Martha copying down a clearance on a notebook. Maybe you could pick that up.

Not a form. Get Gleim's Instrument Pilot Manual it will instruct you how to copy a clearance.

I used C-R-A-F-T when I was an instrument student

C- Clearance Limit
R- Route
A- Altitude
F- Frequency (Departure)
T- Transponder Code

To my knowledge there isn't such a form (unless the FAA has something). Is there a specific something you're looking for?
