its been 3 years and was planning to go to ny this summer

Don't worry about it, leave it in there, because at the border they will ask you about any previous trips into the country. It was the responsibility of the customs official in your country to remove it when you re-entered, the fact that they didn't do it does not matter. This will not prevent you from getting another one, you basically need an I-94 for every trip. When you present your passport at the immigration/customs office when you enter the country, don't say anything, they will find it and ask you about it, just say that when you left the country no one took it out.

Actually it was not the responsibility of another country's immigration service to take your I-94. At that time, 3 years ago, the airline should have taken it when you checked in, then turned it over the DHS/INS. But they often forgot, so your still having it is not anything to worry too much about.

It's too late to turn it in to DHS now if you're traveling to the US this summer, so just leave it in your passport and point it out to the immigration officer when you arrive. If the country you traveled to after the US stamped you in, the officer will see when you left the US, and will know you didn't overstay. If there's no subsequent entry stamp, then find and carry with you a copy of the ticket you used to depart the US, or anything else that would show when you returned home.
