A Caesar uses Clamato brand mixed juice instead of Tomato Juice, and doesn't include some of the other stuff.
Caesar: Highball glass, ice. 1 1/2 oz vodka, fill with Clamato, add 3 dashes Tabasco, 3 dashes Worcestershire
Bloody Mary: Old Fashioned glass. Shake 1 1/2 oz vodka, 3 oz tomato juice, dash lemon, 1/2 Tsp Worcestershire, 2-3 drops Tabasco over ice; strain into glass. Salt/pepper to taste, garnish with lime wedge.

Bloody Mary is tomato juice and Bloody Caesar is Clamato juice. It is extremely difficult to find a Caesar in the United States. In fact, in the southern states, if you order a Caesar, you'll end up with the salad (I tried that once in Florida).

the differences are the alcohol involved and you should use a straw.
the reason to use a straw is to avoid the spillage of red liquid on your clothing.
