In terms of job satisfaction & respect I'd say a surgeon, but the right cartoon at the right time could make much more money.
You don't particularly need to be able to spell correctly for either job so go for whichever one appeals the most

If the question is for you, then to be a surgeon you need to be able to spell, same goes for a cartoonist .
If you mean their wages then a private health Surgeon will get a lot more than a cartoonist(who usually ae free-lance).

the way america is now, a cartoon creator, because there is very little demand for doctors right now.
