if your hearing is affected, i think it's tympanic membrane perforation...Consult your physician

You might be going too deep with the q-tip and causing the ear to bleed. You may have infection and that is causing the blood on the q-tip. You may have an ulcerated sore in the ear that is bleeding when touched by the q-tip. Only way to know is have a doctor look. Since it could be any of the things listed above or even something else. Better to be safe and see a doctor than just guess and end up in a mess later on.
