Its a marketing gimmick.
I am sure at one point some ketchup genius decided to out do the competition by making his refined tomatoes not mere ketchup, but "fancy ketchup". It probably was such a hit all his fellow ketchup makers followed suit.

what makes it fancy is that the ketchup is made with no chunks or anything. which is whatt you get if you try to make it yourself. hence ketchup with no chunks is concidered fancy. even if by todays standards thats nothin special.

I think it probably has to do with the texture or contents.
Some cheap ketchup is all watery and oily and gross.
Fancy ketchup has pizazz and flavor.

I agree, nothing at all fancy, it's for hot dogs, burgers & fries. Oh, and meat loaf. But really, it's just a marketing gimmick.
