because they pay minimum wage to people who really don't care if you get the right order or not

My local Bell gets it right 97% of the time, which is an 'A' grade in school. Besides, where's the motivation to perfectly fulfill every single order? You don't get medals for that, or pay raises either, I'd bet. Perhaps this is why if you were to examine a roster of employees at any given Taco Bell and check back in exactly a year, you'd find some serious turnover. But again, the local Bell in Bethlehem, PA gets great grades from me... visit it sometime.

There could be many reasons, take your pick:
1. They aren't getting paid enough to think.
2. The computer system is malfunctioning/they are not familiar with it.
3. The worker is distracted.
4. The person gathering the items for your requested order may not be the person making the items. That person (the cook, etc.) may not be paying attention...i.e. operating on autopilot.

My local Taco Bell is horrible and they take forever. Even you don't get paid as much as you would like you should still do a good job.

OMG...........and I thought there was a conspiracy against me!!
LOL j/k
But really, they get my order wrong 5 times out of 10. That's NOT acceptable. What kind of standards do they have? Damn them, for having good tacos!! Otherwise, I'd never go there again!
I constantly call and let them know they've screwed up my order, and time after time, they were supposed to send coupons for free food. Never got any of them.

Geez, I have that same problem! Did you know that they have a timer when you pull up and they only have one minute to get you your food? If they're average stays within one minute for the month then they get some kind of bonus... that's probably why they can't get the "dang" order right... because they try to rush all the time... It's ridiculous :)

Because in a fast paced environment there are alot of moving parts, and when there are alot of moving parts in an organization, it's hard to get everything done exactly as it should be done all the time.
Mistakes shouldn't happen, but they should be tolerated once in a while.

I don't know why but there is nothing like getting home and not having all your food or not the right thing.
They aren't taking time to look or they don't care!
