it depends on the case,
but if u mean segregation, u have to reach the sound concrete , then apply special materials from Fosroc, Grace or whatever

Remove any loose material and dust/dirt. Prepare area to be patched with a bonding agent. Mix up your concrete to be used for the patch, make sure you keep your aggregate size less then the thickness of the patched area. Place the concrete and follow manufacturer's directions for curing. Keep traffic off the patch for minimum of 3 days as the patch cures.

1.hammer out the weak chunks
2. remove the trash.
3. groom off the dust
4. spray water on the surface
5. apply cement slurry.
6. pour in concrete( sometimes pressure grouting may be required)
7. cure the finished surface.

and now the surface is ready for use.

Break out the weak concrete around the crack or hole. Pour more concrete.

Put more concrete into the holes/blemishes.
