I have 3 of them actually. One on the computer, a custom built program someone did for me. I love it! It has all I need in it and he adds something if I need it. Two ones I made. I made the covers and all of it for them. One is like a journal that I add what I want, the other has the spells and rituals I use or share. I add all kinds of things to it, pictures, drawings, poetry, etc. I decorate it as I feel like it.

currently it is an icon on my desktop...I type out all of my rituals & such on the computer, so it seems a logical place... i haven't printed it out yet (probably will around samhain)... i will probably just use a loose leaf binder and some nice resume paper, cover the three ring in a leather sleeve (like a "dust jacket)... my last full bos has a pic of a fairy that my friend drew for me along time ago on the front & photocopy of the wheel of the year slipped into the clear plastic sleeves of a white 3 ring binder, the one before that was a black 3 ring binder that has a pentacle drawn on the cover in silver prismacolor pencil, been a witch a long time...the ones before that were in pre bound journals or "blank books" but i didnt like that because they are difficult to arrange...
