No one knows for sure, but the most likely answer is that it was spread by the polio vaccine. Yes, really.
When they found the vaccine for polio, they shipped it off to different places across the world, except they didn't ship large crates of it, (that wouldn't make much sense in the end). They would just ship about 100 ml. Then doctors at the places where they shipped it would make more of it using tissue cultures. Now most people say that the belgian doctors in Africa used tissue cultures from monkeys with the hiv virus. This vaccine, then was used by about 250,000 to 1,000,000 africans, and then that was soon spread across the world.

Aids, (not HIV) happens when the HIV virus overwhelms the immune system. Lots of people with HIV don't have AIDS. However, repeated stresses and exposure of the virus to mucus membranes/anal sphincter area over extended periods may result in a heavy viral titer and mediate destruction of the immune system resulting in the appearance of other pathologies normally suppressed by a healthy immune system.

HIV, the virus that causes AIDS, is very similar to a virus seen in African monkeys, but no one is really sure how it spread to humans at this point...or if they're related at all...
