The short answer is... you can't. There used to be mileage middle-men (for lack of a better term) that allowed you to transfer from one programme to another, but the cost (in miles lost) was significant, and I don't think they're around anymore.

A hopeful answer is... maybe. Because of airline alliances, flights on partner and codeshare airlines can often be booked with each others' miles. For example, Skyteam--which includes Delta, Air France, KLM, Continental, and Northwest (among others). I have booked flights using my Northwest miles on flights actually operated by Delta and Continental.

One World (BA, American, et al) and Star Alliance (United, USAir, SAS, Lufthansa, SAS, et al) also offer the ability to book on another airline--as long as your airline 'codeshare' or allow reward travel redemption on each others' flights.

You can only do so if the airlines you travel with are from the same alliance group., like Star Alliance or such. Think there are 3 such groupings. Best is to check with the airline you favour and see if they accept air miles from which airlines.
