Well there are 2 things. A knock-down, the pressure wave from the blow temporarily shuts-down the inner ear, the balance control center. A knock-down, unlike a knock-out, does NOT injure the brain. Basically, there is an important part of the brain called the thalamus which seems to be a kind of Grand Central Station for imput to the brain from the senses and is also a kind of pivot point for consciousness. It is extremely sensitive to disruption. In one book about the brain I read it said that directly touching it with a feather would render someone unconscious. Fortunately it is located right about at the center of the brain where it's protected from direct injury. In any case where a person is "knocked out" their brain has been distorted enough to impinge on the thalamus.

There is a shock between the brain and the brain stem or between the left and right hemisphere of the brain. This is why an upper-cut on left or right will knock you out as well as a hook which hits the chin across the cheek causing a temporary shock at the brain stem.

Essentially it is a concussion, the brain shifts inside the skull which can cause a person to temporarily be put in a situation where he or she is unable to function normally and/or properly and can loose conciousness in the most extreme cases. When a person gets a head injury, the brain can slosh around inside the skull and even bang against it. This can lead to bruising of the brain, tearing of blood vessels, and injury to the nerves. Again, when this happens, a person can get a concussion - a temporary loss of normal brain function. This can be as minor as just acting as if in a stupor or in extreme cases actually loosing conciousness for a finite period of time. Most people with concussions recover just fine with appropriate treatment which can be as simple as rest and ice to reduce swelling to extreme cases where surgery with medication and monitoring may be required to relieve pressure that builds between the swollen brain and the skull as this can develop into serious life threatening complications. But it's important to take the proper steps if you suspect a concussion, seek medical attention, as this can be extremely serious and may lead to permanent damage which manifests itself in many different ways..
