Every county in the U.S. has a federally mandated County Veterans Service Office. They will have the forms you need to request any document form your official military record. You can find them in your local phone book. Furthermore; you can access the VAs web site at www.va.gov or call from anywhere in the country 1-800-827-1000. This is the numbed for your local office of the regional headquarters of the Veteran Administration.

Go to ur local VA and they will either have the forms or give you a form to fill out to request it...
