Not anymore than it confuses me why we drive on parkways and park on driveways, or why things on a car are called a shipment, but things on a ship are called cargo.

I think a doctor means his "physician practice", which is basically the same thing as "doctor's office". Not that he's practicing on patients.
Like a locksmith would call his business a shop (locksmith shop), physicians and lawyers usually call their businesses practices.
