Why is it that on the back of a medicine bottle it says "adult" is 12 and above, but the adult age in reality is 18?

The "adult" age of 18 is the age that most governments adopted to ensure that there would be an age to determine a person's maturity and readiness for adulthood. Now scientists are finding that the average age of a person's peak maturity is 23.

The medicine bottle refers to those who have gone through puberty. Physically, a person becomes and adult after puberty and their hormonal balance is beginning to develop. Many medicines can conflict with hormonal levels in a person's body which cause negative side effects.

really the illegal age for an adult i 18 but the mental state is at a younger age (most of the time the dosage is lower for children under 18)

For the most part a 12 year old is about the size of an adult. Most pharmaceuticals take this into consideration when accounting for dosage portion.
