I've heard a number of explanations as to why a cat purrs. Some think they purr because they are hurt or sick. This stems from observations taken during cats giving birth...supposedly they excessively purr during contractions and the during the actual birthing process. More widely believed is a cat purrs when he/she is content and/or dreaming during sleep.

With 33 yrs. of experience with cats and kittens they purr when they are happy and they purr when they are content. Mother cats will purr when they feed their babies her milk and so then her babies know the sounds of their mother. Sometimes if a female cat is in heat she may purr while approaching a male cat that she is interested in.

Cats are no different than people. People aren't happy all the time, well the same is true with cats.

Good luck with your cat and I hope I was some what helpful.

(sorry couldn't resist)

I have heard some cats purr when they are in pain, my old(deceased cat) purred more when she was getting sick purred more than she ever did before. But each cat is different. No one knows all the hows and whys of the purr. but I like to hear it from my cat I have now.

Most experts aren't even sure why or how they physically do it.They do purr mainly to express affection. But some also purr when they are sick.
